Enter mg per dose and doses per day. Transdermal fentanyl is entered in mcg/hr (patch strength) and counts once. For a combination product, enter only the opioid component (e.g. hydrocodone 5 mg in hydrocodone/acetaminophen 5/325). For PRN orders, entering the maximum doses per day shows the highest possible exposure; CDC does not specify a convention, so say which you used when documenting.

Not a conversion tool. CDC: do not use the calculated MME to decide doses when converting one opioid to another. When converting, the new opioid is typically dosed substantially lower than the calculated MME because of incomplete cross-tolerance and individual variability.

Scope (CDC 2022): outpatient adults with acute, subacute or chronic pain. The guideline does not apply to pain from sickle cell disease, cancer-related pain treatment, palliative care or end-of-life care. The recommendations are voluntary and "should not be applied as inflexible standards of care across patient populations."

Not included: buprenorphine for pain (partial agonist; excluded from the CDC table), any product for opioid use disorder (CDC: the factors should not be applied to OUD dosing decisions), and parenteral (IV/IM) doses, for which the CDC table gives no factors.
